Simpson had playing spells at Manchester City, Oxford United, Wolverhampton Wanderers and Derby County (where he captained the team to promotion to the Premiership), plus loan spells at Sheffield United and Walsall, then Blackpool and Rochdale (where he was their player-manager from May 2002), until finally hanging his boots up with a cameo appearance for Carlisle United against Stockport County on May 6, 2006.
Simpson represented England at under 21 level.
Simpson managed Carlisle United football club to the Football League Two Championship in the 2005/2006 season, a second successive promotion following Carlisle's promotion from the Nationwide Conference in 2004/2005.
In June 2006. Simpson replaced Billy Davies as manager of Preston North End and led the Lancashire side to the top of the Championship by December, their highest league placing for 55 years. However, the club failed to make the end-of-season play-offs, finishing in 7th place after losing 7 of their final 8 games.
A bad start to the 2007-08 season which saw Preston pick up just 3 victories led to Simpson being sacked on 13th November 2007.
